Volunteers – Please complete both SECTION A and SECTION B. SECTION C is optional.  Completed paperwork is required BEFORE you volunteer.

Thank you for your interest in volunteering at Capilano Elementary. Please review each section below for important volunteer instructions and paperwork that is required.  

SECTION A: SCHOOL BASED VOLUNTEER FORMS – Need to be completed each school year. This package is available to pick up at the main office.

(3) Confidentiality Declaration

 When completed, please return these forms in person to the Capilano School office. 

SECTION B: POLICE INFORMATION CHECK  (PIC) – valid for 5 years.

A completed Police Information Check is required from you prior to your participation.

Please visit the NVSD online Police Information Check for Volunteers website.

If you reside in North Vancouver, your PIC is processed free of charge when you fill in, print out and present the forms provided on this website to the local RCMP detachment.  Please complete your PIC as soon as possible at the RCMP or Police detachment where you reside. 

Please pick-up your processed PIC from the RCMP/Police detachment and return the original copy to the NVSD Human Resources Department (HRD), located at 2121 Lonsdale Avenue, North Vancouver.  Once you have delivered your original PIC to the Human Resources Department at NVSD please let the Capilano office (capilano@sd44.ca) know so we can then follow up with them to confirm your approval. 

SECTION C: VOLUNTEER DRIVER – valid for 3 years

*disregard if you are not driving students 

If you are planning to drive students, other than your own, please complete the following… 

Driver Requirements:

• Driver's Application Form (available on the Police Information Check for Volunteers website)
• Obtain your Personal Driving Record (instructions below)
• Police Information Check (see SECTION B above for PIC information)
• Please bring your current insurance papers and driver's license to the main office for us to photocopy
